SHAPE

SHAPE is an immediate re-entry model of services for those who have been incarcerated as well as those who are otherwise Criminal Justice involved through Diversion Courts, Probation, Parole, etc.

The track will offer:

• Outpatient treatment and MAT (Medication Assisted Treatment) for those who have a history or on-going issue with Substance Use Disorder.

• Mental health services for those with co-occurring disorders.

• Health Home Care Management support to assist with the coordination of appropriate housing for those re-entering and/or requiring re-placement  to assist with their goals related to Probation, Court, etc., as well as the monitoring of successful relocation once settled.
